Job summary

Family Health Centers of San Diego (FHCSD) seeks an HIM Abstractor to support record analysis, data abstraction, and targeted quality measurement efforts. Under the HIM Manager and CIO, you will contribute to chart analysis, QA/QC activities, and archive management within a collaborative team environment.

The role requires knowledge of medical terminology, EHR systems, and coding conventions. Strong written and verbal communication skills are essential, with bilingual English/Spanish desirable.

Qualifications

• At least 2 years of experience in Medical Records or HIM operations.
• RHIA or RHIT certification preferred.
• Proficient with databases and data abstraction work.

Responsibilities

• Abstacts clinical data from medical records for QA/QC and clinical quality measurement reporting.
• Assist in design and implementation of HIM training modules (document management, PHI handling).
• Communicate significant findings to Clinical Quality or Utilization teams in a timely manner.
• Maintain documentation of gaps identified during medical record reviews for practice location education.
