Family Health Centers of San Diego (FHCSD) is seeking a Health Information Management Specialist II to handle chart preparation, record requests, and quality assurance across multiple clinic sites. You will ensure accurate medical records, proper PHI handling, and timely data entry while delivering excellent customer service to clinical staff.

This role requires a High School diploma, a medical terminology course, and familiarity with RHIA/RHIT or related certifications is preferred.

Qualifications

• High school diploma or GED required.
• Medical Terminology course required or completed within 90 days.
• Medical Assistant Certification preferred.
• RHIA or RHIT Certification preferred.

Responsibilities

• Prepares patient charts for upcoming visits, reviewing for open orders and pending results.
• Coordinates release of medical information following guidelines.
• Verifies authorizations and processes requests; communicates with requestor.
• Provides customer service for the department by phone, email, and in person.
• Reviews medical records for accuracy and completeness.
• Participates in QA/QC and data abstraction as assigned.
• Produces accurate work in a high-volume environment.
• Adheres to HIPAA and privacy rules for PHI.
