Qualifications

• RN with active, unrestricted Michigan license; BSN not required.
• At least 1 year of clinical RN experience.
• Current CPR certification.

Responsibilities

• Provide home-based care to patients with illnesses, injuries and chronic diseases.
• Oversee LPNs, CNAs and aides delivering patient care.
• Assess patients, document progress and report changes to physicians.
• Educate patients on plan of care, medications, therapy, diet and exercise.
• Coach families on the care plan and home safety.
• Coordinate discharge from services to ensure smooth transitions.
