Interim HealthCare of West Michigan is expanding its Skilled Home Health Services into the Kalamazoo area. This RN Case Manager role offers a salaried position with weekly bonus opportunities and a robust benefits package.

You will provide in-home nursing care, coordinate with physicians and supervise LPNs/CNAs, while promoting autonomy and work-life balance for a dedicated team of clinicians.

Qualifications

• Must hold an active, unrestricted RN license in Michigan.
• Minimum of 1 year of clinical RN experience required.
• Home health experience is a plus.

Responsabilités

• Provide home-based care to patients with illnesses, injuries, and chronic diseases.
• Oversee LPNs, CNAs and aides providing patient care within a team.
• Educate patients on plans of care, medications, therapy, diet and exercise.
• Coach families on care plans, safety and home routines.
• Coordinate discharge planning and communicate progress to physicians.
