Job summary

Advocate Health in Georgia seeks a Hematology/Oncology Clinical Pharmacist to provide direct patient care within the Infusion Oncology setting. You will advise on oncology treatment plans, support transitions of care, and collaborate with nurses, physicians, and pharmacy staff to ensure medication access and safety.

The role emphasizes education, clinical review of care plans, and opportunities to precept residents and students within a busy cancer center.

Qualifications

• Licensed in the state of Georgia or eligible for reciprocity
• Immunization and non‑vaccine injectable certification required based on area of practice
• Credentialing and privileging required based on area of practice
• PGY2 Oncology Residency or strong Oncology experience

Responsibilities

• Oversees pharmaceutical care, reviewing literature and designing regimens
• Provides patient education on therapy, adverse effects, and adherence
• Monitors and evaluates patient-specific pharmacotherapeutic plans
• Interprets medication orders and supports transitions of care
• Collaborates with healthcare team to manage formulary adherence and med-use processes
• Prepares and dispenses medications in compliance with regulations
