Job summary

Qualifications

• Doctorate of Pharmacy plus one year residency is required.
• Licensed by the Georgia State Board of Pharmacy.
• Minimum PGY-1 training plus 1 year clinical experience OR PGY-2 training OR 3-5 years of equivalent experience.

Responsibilities

• Identifies and resolves medication-related problems through rounds, profile reviews and consult management.
• Applies knowledge of medication management and patient assessment to ensure safe, evidence-based therapy.
• Collaborates with Medication Safety to prevent adverse effects and outcomes.
• Supports antimicrobial stewardship by ensuring formulary compliance and guidance on prescribing.
