Qualifications

• BLS certification within 90 days; ACLS if applicable to practice specialty.
• ASHP PGY1 residency or board certification preferred.

Responsibilities

• Prevent and resolve drug-related problems by consulting with prescribers and documenting interventions.
• Monitor drug therapy regimens and adjust per protocols; document all activities.
• Engage with clinical pharmacists and leadership for complex cases.
• Educate patients using effective education materials and improve adherence.
• Serve as preceptor or educator for students and residents; participate in QI activities.
