Advocate Health is seeking a Hematology Oncology Clinical Pharmacist to provide direct patient care and support patients referred to the Department of Oncology. The role includes embedded clinic time in WT Infusion Oncology and advising as a clinical expert for patients in the acute care setting.

Responsibilities include patient education, clinical review of oncology care plans, drug information support for outpatient and inpatient units, ensuring medication access during transitions of care,

Formación

• Bachelor’s or Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy or NABP FPGEC certification.
• PGY2 Oncology Residency or strong Oncology experience.
• Licensure/credentialing requirements per Georgia practice.

Responsabilidades

• Oversees pharmaceutical care and designs medication therapy regimens based on biomedical literature.
• Provides patient education on medication therapy, adverse effects, compliance, and administration.
• Reviews medical orders and participates in medical emergencies when applicable.
• Supports medication safety, quality improvement, and formulary adherence.
• Precepts pharmacy students and mentors pharmacy teammates as assigned.
