Inova Health System invites applications for a Transplant Coordinator – Post Heart at Inova Fairfax Hospital. This full-time role includes standard hours, on-call weekends, and remote triage. The coordinator will work with an interdisciplinary team to optimize patient care, ensure regulatory compliance, and contribute to continuous quality improvement in transplant services.

Qualifications

• Bachelor's degree in nursing is required.
• Three years of RN experience.
• Licensed in the Commonwealth of Virginia as a Registered Nurse; BLS certification.

Responsibilities

• Assist with patient care planning and coordination with the health-care team.
• Document plans/orders/outcomes per policy.
• Delegate patient care activities for caseload management.
• Educate patients, families and care providers on care aspects and plans.
• Ensure data submission and reporting accuracy for compliance and research.
• Participate in program quality assessment and improvement initiatives.
• Collaborate with interdisciplinary team to achieve patient outcomes.
• Maintain ongoing education and certifications; perform other duties as assigned.
