Job summary

Sentara Heart Hospital in Norfolk, VA is hiring a Transplant Coordinator Registered Nurse for the Advanced Heart Program. The coordinator guides the complex logistics of heart transplant and LVAD care, prioritizing patient safety and a smooth donor-recipient journey.

You will manage transplant evaluations, care pathways, and follow-up, provide clinical leadership to the care team, and coordinate multidisciplinary planning to optimize outcomes for patients with advanced heart failure.

Qualifications

• BSN required.
• Virginia RN license or compact/multi-state eligible.
• BLS within 90 days of hire.
• Certified Clinical Transplant Coordinator (CCTC) within three years of hire.
• Two years of related nursing or transplant experience.

Responsibilities

• Coordinate transplant/VAD evaluation, process, and follow-up care.
• Provide clinical leadership to care team members and manage outcomes.
• Facilitate donor and recipient journey with emphasis on patient safety.
