Job summary

True North Consultants, Inc. is seeking a Cardiologist (Heart Failure Specialist) to provide inpatient and outpatient heart failure care, lead a dedicated HF team, and drive program development in Indiana.

The role emphasizes GDMT, remote monitoring, and collaboration with Structural Heart and EP teams. Strong leadership and state licensure are required; competitive base salary and incentives accompany a comprehensive benefits package.

Qualifications

MD or DO with ACGME-accredited Internal Medicine residency and Cardiovascular Disease fellowship.
Fellowship training or specialized clinical focus in Advanced Heart Failure & Transplant Cardiology.
Board Certified in Cardiovascular Disease; BC/BE in Advanced Heart Failure.
Active or eligible for state medical license, DEA, and state controlled substance registrations.

Responsibilities

Provide comprehensive heart failure management across inpatient and outpatient settings.
Diagnose and treat complex cardiomyopathies and advanced heart failure using GDMT.
Manage remote monitoring and outpatient therapies to prevent decompensation.
Perform diagnostic workups including echocardiography, nuclear imaging, and right heart caths.
Lead HF Advanced Practice Providers within a collaborative practice model.
Collaborate with Structural Heart, EP, Rehab, Palliative Care, Dietitians, Social Work.
Lead quality initiatives, readmission reduction, GDMT optimization, and pathways.
Work with a broad network of cardiologists, PCPs, and hospitalists to streamline care.
