Top Echelon Software, LLC. is seeking a Cardiologist (Heart Failure Specialist) in Indiana to lead inpatient and outpatient heart failure care, guide advanced therapies, and coordinate across multidisciplinary teams.
The role emphasizes GDMT, remote monitoring, device management, and collaboration with Structural Heart and EP services. Board-certified physicians with an Indiana license or eligibility are encouraged to apply.
Qualifications
- MD or DO with ACGME-accredited Internal Medicine residency and Cardiovascular Disease fellowship.
- Fellowship training in Advanced Heart Failure & Transplant Cardiology.
- Board Certified in Cardiovascular Disease; BC/BE in Advanced Heart Failure.
- Active or eligible for Indiana medical license, DEA and registrations.
Responsabilités
- Provide inpatient and outpatient heart failure management across services and clinics.
- Diagnose and treat complex cardiomyopathies using GDMT and modern protocols.
- Manage remote monitoring and IV diuretics; oversee implantable devices when needed.
- Perform echocardiography, nuclear imaging and right heart catheterizations.
- Lead Heart Failure NPs/PAs within a collaborative team model.
- Collaborate with Structural Heart, EP, Rehab and other teams for holistic care.
- Drive quality initiatives and hospital readmission reduction programs.
