Job summary

Hackensack Meridian Health seeks a Nurse Navigator for the Structural & Congenital Heart Program. The role guides patients from initial consultation through preoperative prep, admission, and postoperative evaluation, coordinating care across APNs/MDs and staff.

Ideal candidate is an RN with 5+ years of relevant experience, AACN-accredited nursing education, and strong communication and computer skills. NJ licensure required; emphasis on education for patients and families.

Qualifications

• RN with at least 5 years of clinical experience in a relevant clinical specialty.
• Graduation from NLN/AACN accredited program in nursing.
• Proficient computer skills (MS Office and/or Google Suite).
• Strong organizational, time management, and critical thinking RN skills.
• Ability to work independently and exercise sound judgment with physicians and care teams.
• Excellent written and verbal communication skills.

Responsibilities

• Provides competent clinical care to patients during office visits.
• Provides education and educational resources to patients and families for optimal outcomes.
• Ensures materials, lab reports, and diagnostic studies are available for review prior to visits.
• Assesses patient data and coordinates scheduling to ensure timely care.
• Monitors medical records to maintain outcomes per standards of care.
• Mentors and supports ancillary staff in outpatient settings.
• Participates in follow-up calls and postoperative care tasks.
• Adheres to organizational competencies and standards of behavior.
