Job summary

Enhabit Home Health & Hospice is seeking a dedicated relationship manager to cultivate referral partnerships with physicians, hospitals and health systems in Florida. You will educate partners on services, drive referral growth, and assist families through post-acute transitions to improve patient outcomes.

The role requires a healthcare-related degree, 2+ years in healthcare business development or sales, and strong communication skills.

Qualifications

• Bachelor's degree in healthcare administration, marketing or related field, or nursing/social work degree.
• Minimum two years in healthcare business development, community relations, marketing, or sales.
• Knowledge of Medicare home health/hospice operations and regulatory guidelines.
• Valid state driver's license and auto liability insurance.
• Excellent interpersonal, communication and presentation skills.
• Clinical license preferred if applicable.
• Strong ability to travel and attend in-person meetings regularly.

Responsibilities

• Serve as relationship manager between Enhabit and referral sources including physicians and hospitals.
• Support referral growth by educating sources on services and program requirements.
• Navigate post-acute care transitions to improve patient outcomes and satisfaction.
