Job summary

Buckeye Commons Care & Rehab in Kent, Ohio is seeking a Healthcare Community Liaison who will build relationships with hospitals, physician offices, and other referral partners to drive referrals and expand market share.

You will educate partners on our post-acute care services, develop territory development plans to increase census and revenue, identify new opportunities, and work with discharge planners to connect patients to the right level of care.

Qualifications

• Background in healthcare sales or business development.
• Experience with referral partners, hospitals and physician offices.
• Proven track record in territory development and census growth.
• Excellent communication, organizational and problem-solving skills.
• Ability to work independently and as part of a team.
• Strong networking and community outreach abilities.

Responsibilities

• Build and maintain relationships with hospitals, physician offices, and other referral partners to generate referrals and grow market share
• Develop and execute a territory development plan to increase census and revenue
• Identify opportunities to expand services and partnerships in the community
• Cultivate relationships with case managers, discharge planners, and other healthcare professionals
• Develop and maintain understanding of post-acute care options and services
• Provide education and training to referral partners on services and benefits
• Identify and capitalize on opportunities to improve patient outcomes and reduce healthcare costs
