Job summary

Phoenix Med Health is seeking a Healthcare Account Manager to own and grow the referral network across assisted living facilities, home health agencies, hospice, and other partners for Active Vital Care.

In this field-based role, you will visit partners regularly, build trust with clinical and administrative stakeholders, and drive steady referrals and patient registrations through disciplined pipeline management.

Qualifications

• 2+ years in healthcare account management, business development, or referral management.
• Experience with facilities, home health, hospice, or similar referral environments.
• Comfortable being in the field regularly.
• Strong communication and relationship management skills.
• Highly organized with the ability to manage a high-volume partner pipeline.

Responsibilities

• Own and grow relationships across facilities, home health agencies, hospice, and other referral sources.
• Represent Active Vital Care in the field as the primary point of contact.
• Drive consistent patient referrals and new patient registrations.
• Conduct regular in-person visits to partners.
• Identify and onboard new referral partners.
• Track outreach, referrals, and pipeline activity in CRM.
• Coordinate internally with scheduling, clinical, and operations teams.
• Resolve issues quickly and maintain strong partner satisfaction.
• Continuously increase referral volume per account.
