Prominence Health in South Texas is seeking a Performance Improvement Coordinator to support operational activities and process improvement initiatives under the Director of Performance Improvement. This on-site role will involve field activities across the South Texas market, focusing on risk adjustment, HEDIS quality initiatives, medical records projects, and provider support.

You will coordinate field operations, assist with program deployment, maintain documentation, and deliver routine

Qualifications

• Bachelor’s degree in business, health administration or other related field.
• Minimum of three years of experience in process improvement, project management, quality management or related field.
• Ability to effectively communicate in English, both verbally and in writing.
• Organized, able to meet deadlines and versed in project management.
• Able to collaborate across department and with external partners including provider groups.
• Passion for improving member experience and outcomes.
• Experienced in analyzing large sets of data.
• Proficient in Microsoft Suite with proficient skills in Excel.
• Experienced in healthcare, preferably in a managed care setting.
• Preferred - experience working with medical records, clinical settings, and familiar with Medicare risk adjustment.
• Excellent verbal and written communication skills.
• Ability to establish and maintain positive and effective work relationships.

Responsibilities

• Coordinate field operations and support performance improvement programs.
• Assist with risk adjustment, HEDIS quality initiatives, and medical records projects.
• Support provider-facing activities and performance-improvement initiatives across South Texas.
• Prepare routine program reports and status updates for leadership and partners.
• Maintain operational documentation and track program progress and outcomes.
