Job summary

Praxis Health is seeking a Quality Improvement Manager to lead HEDIS performance, payer incentive programs, and population health initiatives. You will guide quality opportunities, monitor payer programs, and develop strategies with a team supporting quality and performance.

As the EMR subject matter expert, you’ll ensure workflows, documentation, data capture, and reporting align with quality goals, partnering with Population Health leadership to implement sustainable improvements.

Qualifications

• Bachelor's degree in healthcare administration, nursing, public health, HIM, or related field preferred.
• 3–5+ years of experience in healthcare quality, HEDIS, population health, or payer quality programs.
• Experience managing or supervising a team.
• Strong knowledge of HEDIS measures, quality metrics, care gaps, and payer incentive programs.
• Experience with EMR systems and quality-related functionality, reporting, registries, and workflows (Cerner/Oracle Health preferred).
• Strong analytical skills to translate quality data into actionable improvements.
• Excellent communication, presentation, and organizational skills.

Responsibilities

• Lead the organization’s quality improvement initiatives focused on HEDIS performance and payer programs.
• Monitor and analyze performance across quality metrics to identify trends and opportunities.
• Develop and implement interventions to close care gaps and improve outcomes.
• Direct and develop a team supporting quality and payer incentives.
• Serve as EMR quality expert for workflows, documentation, and reporting.
• Collaborate with Population Health and other leaders to implement sustainable improvements.
