Job summary

Synergy Home Care in Chicago seeks a Healthcare Marketing & Business Development Representative to expand our referral network and increase client census through established professional relationships. This commission-based role focuses on driving referrals from case managers, social workers, discharge planners, physicians, and community partners.

You will educate referral sources about our services and care model, attend healthcare events, and collaborate with intake to ensure smooth onboarding

Qualifications

• Minimum 2–3 years of healthcare marketing or business development experience.
• Established relationships with case managers, social workers, or discharge planners.
• Proven ability to bring in Medicaid, Medicare, and/or Private Pay clients.
• Strong understanding of home care services and referral processes.

Responsibilities

• Develop and maintain referral relationships with Medicaid/Medicare case managers, discharge planners, hospitals, rehab facilities, and social workers.
• Actively market Synergy Home Care services to professional and community partners.
• Generate qualified referrals and convert leads into active clients.
• Educate referral sources on services, programs, and care model; attend networking events and healthcare functions.
• Track referral activity and report outcomes to agency leadership.
• Collaborate with intake and administrative teams to ensure smooth onboarding of new clients.
