Job summary

Quality Home Health, Hospice & Palliative Care in Holladay, UT is seeking a full-time Intake Coordinator to manage referrals and admissions for our home health and hospice services. This in-office role offers a Monday-Friday schedule, 9:00 AM–5:00 PM, with benefits.

You will coordinate new referrals, communicate with referral partners, verify information, and ensure a smooth transition into care while maintaining HIPAA privacy and excellent service.

Qualifications

• Experience in healthcare intake, referral coordination, or medical office administration preferred.
• Home health or hospice experience strongly preferred.
• Excellent organizational skills and attention to detail.
• Strong verbal and written communication abilities.

Responsibilities

• Receive, review, and process incoming home health and hospice referrals.
• Gather and verify patient information, orders, medical records, and documentation.
• Coordinate with referral sources to obtain missing information and verify insurance coverage.
• Enter and maintain referral and patient data in the EMR; track status and timelines.
• Assist with admissions scheduling and start-of-care coordination with clinical teams.
• Ensure compliance with HIPAA and agency policies.
