Ultimate Staffing Services seeking a Healthcare Intake & Referral Coordinator in Woodbury, MN. This onsite role coordinates referrals, validates insurance, and manages admissions and documentation in the EMR system.

You will communicate with providers, clients, and families, maintain accurate records, and support care coordination activities during a temp-to-hire engagement with regular business hours.

Qualifications

• Associate degree required; Bachelor’s degree preferred.
• Experience in healthcare intake, admissions, or referral coordination.
• Medicare intake experience preferred.
• Home health, hospice, or home care experience preferred.
• Knowledge of insurance verification, prior authorizations, and payor types.
• Familiarity with medical terminology.
• EMR experience and strong computer skills.
• Certified Medical Assistant (CMA) preferred.

Responsibilities

• Coordinate incoming referrals and admissions.
• Verify insurance eligibility and obtain required authorizations.
• Manage client demographics, medical information, medications, and documentation within the EMR system.
• Communicate with referral partners, healthcare providers, clients, and families.
• Process medical orders, referrals, and related documentation.
• Create and maintain client care plans and required records.
• Track referral activity and client data using spreadsheets and internal systems.
• Resolve client inquiries and concerns in a timely, professional manner.
• Assist with scheduling and occasional on-call coverage.
• Ensure accurate and timely documentation to support quality care delivery.

Position Details

Location: Woodbury, MN (100% Onsite)

Schedule: Monday-Friday | Regular Business Hours

Employment Type: Temp-to-Hire (720 hours)

Pay Range: $26-$29/hour
