Urban Health Plan in Harlem is seeking a Specialty Case Manager to support patients through social services, screenings, and referrals. This onsite role focuses on advocacy and connecting patients with needed resources within CitiCares Community Health Center.

The position emphasizes teamwork, medical terminology understanding, and a commitment to delivering high-quality service to diverse communities. Eligible for competitive benefits and a 40-hour week.

Qualifications

• Bachelor's degree in social work, psychology, human services or related field.
• Entry level case management; no prior experience required beyond Bachelor's Degree.
• Team player who supports patients and colleagues.
• Proficiency with Microsoft Office tools.
• Understanding of medical terminology and social work principles.
• Commitment to excellence and quality service to patients.

Responsibilities

• Provide case management assistance to patients as assigned.
• Prepare and provide letters for public assistance, housing, IRS or schools.
• Provide community resources based on site/program needs.
• Advocate for patients to obtain resources or services.
• Collaborate with coordinators and team members to support patients.
• Perform general case work and support additional duties as assigned.
