Position Summary:The Specialty Case Manager (Bilingual English/Spanish) will be a member of our Social Services department, working in support of patients in our Bronx-based health centers. The Case Manager provides social assistance, general screenings to identify support services needed, provides internal/external referrals, advocacy, and social services for our patients. The current opportunity is to work in our of our Bronx Based Family Health Centers with an emphasis on adult medicine services.
Work Environment Type, Location, and Schedule:Onsite, Bronx, NY, Monday to Friday 8:00 am to 5:00 pm. Compensation: $45,560 based on 40 regular hours per week.
