Job summary

HEALTHCARE FOR THE HOMELESS - HOUSTON is seeking a case manager to work closely with clinicians and staff to help consumers access a continuum of care, including housing, benefits, and medical referrals. The role involves intensive case management, information and referral, crisis intervention, and advocacy.

You will build therapeutic relationships, develop negotiated care plans, and coordinate with multiple agencies while maintaining confidentiality and thorough documentation.

Qualifications

• Bachelor’s degree or higher in a related field.
• Minimum 3 years of experience working with homeless or medically underserved populations.
• Proficient with Microsoft Office, Excel, and data-base systems.
• Clinical assessment experience.
• Familiarity with homelessness, substance use, incarceration, and mental illness.

Responsibilities

• Provides support services to consumers with physical health, mental health and/or substance use problems.
• Builds and maintains a therapeutic relationship with consumers and negotiates goals.
• Performs assessments and develops negotiated care plans with consumers.
• Updates care plans and communicates progress to the team.
• Liaises with medical, social services and housing resources for referrals.
• Develops referral networks and coordinates with outside agencies.
• Maintains confidentiality and documentation for all consumers.
• Complies with agency policies and participates in required trainings.
• May travel to offsite destinations for meetings and client coordination.
