Job summary

The New York Foundling is seeking a MAP Coordinator to coordinate medical care for individuals with developmental disabilities, promoting health advocacy and overall well-being. You will manage appointments, transportation, and communication with healthcare teams.

Candidates should have a high school diploma and at least 1 year of related experience; CPR/First Aid is preferred and a valid driver’s license is required. This is a full-time, on-site role in New York.

Qualifications

• High school diploma or equivalent required; healthcare-related certification or degree a plus.
• Valid driver’s license with 6 months experience and 6 or less points.
• Minimum of 1 year of healthcare coordination or related field experience.
• CPR and First Aid certification preferred; disability services/medical administration/crisis intervention training a plus.

Responsibilities

• Coordinate medical care including scheduling primary care, specialist visits, therapy sessions, and diagnostic tests.
• Arrange transportation to medical appointments and accompany patients as needed.
• Advocate for healthcare rights and access for persons with developmental disabilities.
• Maintain accurate medical records and communicate with healthcare providers and interdisciplinary teams.
• Provide information and education on procedures, medications, and preventive care.
