Qualifications

  • High school diploma or GED; healthcare-related certification a plus.
  • Valid driver’s license with 6 months of experience and 6 or fewer points.
  • Minimum 1 year of experience in healthcare coordination or patient advocacy.
  • Strong organizational, communication, and interpersonal skills.
  • CPR/First Aid certification preferred; disability services training a plus.
  • Commitment to compassionate care for persons with developmental disabilities.

Responsibilities

  • Schedule and coordinate medical appointments, including primary care visits, specialist consultations, therapy sessions, and tests.
  • Provide or arrange transportation for persons to and from medical appointments.
  • Advocate for healthcare rights and needs of persons with developmental disabilities.
  • Maintain accurate records of medical appointments, diagnoses, medications, and treatment plans; report findings.
  • Provide information and education on procedures, medications, and preventive care to promote health literacy.
  • Respond to medical emergencies or crisis during appointments and coordinate follow-up care.
