Qualifications

• Experience in skilled nursing facility admissions or healthcare administration preferred.
• Knowledge of Medicare, Medicaid, and managed care payer sources.
• Proficiency with PCC, Microsoft Office, and hospital referral portals.
• Detail-oriented with strong documentation and verification skills.

Responsibilities

• Coordinate and process new resident admissions.
• Respond to inquiries from prospective residents, families, hospitals, and referral sources.
• Receive, review, and organize admissions documentation and referrals.
• Verify insurance information, payer sources, and required paperwork.
• Coordinate with nursing, business office, social services, and leadership for smooth transitions.
• Maintain accurate records and update referral/admissions tracking systems.
• Follow up on pending referrals and missing documentation.
• Communicate admission status and required documents with hospitals and case managers.
• Provide excellent customer service to residents, families, and partners.
