Job summary

Sweetwater Care in Selma, CA is seeking an Admissions/Referral Development professional to build referral relationships with hospitals, clinics, and senior-service organizations to grow census and occupancy. The role focuses on external sales planning, partnerships, and guiding prospective residents and families through the admission process.

The position requires a Bachelor’s degree or state nursing license and at least three years of census development experience, plus a valid driver's license

Qualifications

• Bachelor's Degree preferred; or an unencumbered nursing license in the state.
• Valid driver's license in the state.
• Minimum three (3) years of census development and occupancy management experience in a nursing home or health care facility.
• Knowledgeable of nursing and medical practices, and long-term care laws and regulations.

Responsibilities

• Plans, develops, organizes, and implements an external sales strategy with daily activities to achieve census goals while complying with laws.
• Plans and implements an external census sales strategy to build and maintain relationships with referral sources (hospitals, physicians, senior centers, home health, hospice, etc.).
• Identifies opportunities with new referral partners and develops strategies to build relationships and communicate services.
• Submits monthly calendar of events to supervisor demonstrating census strategy implementation.
• Provides weekly documentation of calendar execution to supervisor.
• Develops and implements a sales strategy that converts referrals to admissions.
• Schedules events with referral partners and reports results to the Admissions Director; follows geographic territory guidelines.
• Maintains knowledge of Medicare, Medicaid and insurances related to long-term care.
