Job summary

Astoria Senior Living, part of Agemark Senior Living, seeks a Health & Wellness Director to lead a compassionate wellness team and provide exceptional resident care in Omaha, NE. You’ll oversee assessments, service plans, and coordination with families and healthcare providers.

Qualifications

• Active Nebraska LPN or RN license is required.
• Minimum of 1 year of experience with seniors or in long-term care.
• 2+ years of leadership or management experience preferred.
• Strong knowledge of medications and senior health needs.
• Excellent communication, organizational, and problem-solving skills.
• Comfortable using electronic documentation and computer systems.

Responsibilities

• Lead and mentor the wellness team promoting a culture of excellence.
• Oversee resident assessments, admissions, service plans, and care coordination.
• Ensure residents receive exceptional, person-centered care.
• Communicate effectively with residents, families, physicians, pharmacies, and providers.
• Supervise medication management and safe administration.
• Maintain compliance with state regulations, licensing requirements, and policies.
• Monitor documentation and care plans for accuracy and compliance.
• Respond to resident concerns, emergencies, and changes in condition.
• Identify risks and implement proactive safety plans.
• Participate in Manager on Duty rotations and provide leadership support.

What You'll Do

• Lead, mentor, and support the wellness team while promoting a culture of excellence.

• Communicate effectively with residents, families, physicians, pharmacies, and healthcare providers.

• Monitor documentation, resident records, and care plans for accuracy and compliance.
• Respond to resident concerns, emergencies, and changes in condition.
• Identify potential risks and implement proactive care and safety plans.
• Participate in Manager on Duty rotations and provide leadership support as needed.

What We're Looking For

• Active Nebraska LPN or RN license required.
• Minimum of 1 year of experience working with seniors or in long-term care, assisted living, or a related healthcare setting.
• 2+ years of leadership or management experience preferred.
• Strong knowledge of medications, treatments, and senior health needs.
• Excellent communication, organizational, and problem-solving skills.
• Comfortable using electronic documentation and computer systems.
