Job summary

GLIDE seeks a Health Systems Navigation Case Manager to provide client-centered case management with a harm-reduction focus for individuals who use substances in San Francisco. You will coordinate access to primary care, behavioral health, HIV/HCV/STI prevention, and social services, aiming to re-engage clients in care and support housing and stability goals.

Qualifications

• Bachelor’s degree in social work, public health or related field, with 2+ years of harm reduction case management experience.
• Extensive knowledge of prevention and treatment of HIV, HCV, and STIs.
• Ability to plan outreach objectives aligned with HEAT program needs.
• Ability to establish and maintain collaborative relationships across GLIDE programs, city agencies, and community partners in SF Bay Area.
• Understanding of core principles of harm reduction and trauma-informed care.
• Strong communication and data entry skills with HIPAA compliance.

Responsibilities

• Conduct client intakes and assessments to identify strengths, needs, and barriers to care.
• Develop, implement, and update individualized care plans in collaboration with clients to achieve health, housing, income, and stability goals.
• Provide ongoing, strengths-based case management and consistent follow-up to a dedicated caseload to support progress toward care plan objectives.
• Coordinate internal and external services, including medical care, behavioral health, substance use treatment, and housing, and assist clients with documentation and benefits.
• Maintain accurate, confidential client records and ensure HIPAA-compliant documentation.
• Complete required data entry in Salesforce and other designated databases.
