Job summary

GLIDE is seeking a Health Systems Navigation Case Manager to provide client‑centered, harm-reduction focused case management for individuals using substances. You will facilitate access to primary care, behavioral health, and HIV/HCV/STI prevention, with outreach in SF and surrounding areas.

Responsibilities include intake, care planning, documentation, and coordination with partners to re‑engage clients in care and housing resources. Strong communication and trauma‑informed skills are essential.

Qualifications

• Bachelor’s degree in social work, public health or related field.
• Extensive knowledge of prevention and treatment of HCV, HIV, and STIs.
• Ability to plan and implement outreach objectives for HEAT program needs.
• Ability to collaborate across GLIDE programs, city agencies, and community partners in SF and the Bay Area.
• Understanding harm reduction and trauma‑informed care principles.

Responsibilities

• Conduct client intakes and assessments to identify strengths, needs, and barriers to care.
• Develop, implement, and update individualized care plans with clients to achieve health, housing, income, and stability goals.
• Provide ongoing case management and follow-up to support progress toward care plan objectives.
• Coordinate internal and external services, including medical care, behavioral health, housing, and benefits access.
• Maintain HIPAA-compliant records and complete data entry in databases like Salesforce.
