Job summary

GLIDE in San Francisco seeks a Health Access Navigator to provide harm reduction supplies and health navigation for people who use drugs. You will support syringe access, link clients to services, and assist with data collection and reporting, contributing to the DOPE and Narcan programs.

You will engage clients with motivational interviewing, coordinate outreach activities, and maintain records while upholding HIPAA confidentiality and professional boundaries.

Qualifications

• Direct experience delivering harm reduction services (syringe access, outreach, and health navigation).
• Experience providing direct social services to homeless or very low-income clients who are multiply diagnosed or involved with criminal justice system.
• Knowledge of violence prevention, de-escalation, and motivational interviewing practices.
• 1–2 years harm reduction, case management, or advocacy experience with marginalized populations.
• Experience serving clients who are dual diagnosed or have domestic violence history or incarceration.
• Strong writing and documentation skills are essential.
• Excellent interpersonal/communication skills and ability to maintain relationships.
• Experience with crisis management and de-escalation in individual and group settings.
• Ability to work independently and meet client needs while adhering to confidentiality.

Responsibilities

• Provide harm reduction supplies and safe-use materials at SAS desk and during outreach.
• Promote syringe access and HIV/Hep C services, plus other health and recovery services.
• Assist SAS Manager with data collection and monthly reporting.
• Support Community Engagement Coordinator in training and hours for navigators.
• Deliver group education, support groups, and outreach materials.
• Provide prevention and health education including safer sex and safer injection practices.
• Assist with Narcan distribution and overdose prevention efforts.
• Coordinate community outreach activities and events to promote testing and care links.
• Build rapport with clients using motivational interviewing for transitions to long-term services.
• Maintain program data records, progress notes, and documentation in the system.
