GLIDE in San Francisco seeks a MOUD case manager to connect clients with opioid use disorder to methadone, Suboxone, naltrexone and other medications, while assisting with housing, identification, and mental health resources to support stabilization and self-sufficiency.

This full-time role requires outreach, education, and compassionate engagement with clients, along with documentation and adherence to HIPAA protocols.

Qualifications

• Bachelor’s degree in social work, public health or related field OR 2+ years providing MOUD navigation and harm reduction focused case management services.
• Experience in overdose reversals, de-escalation techniques, motivational interviewing.
• Understanding of social determinants of health and their impact on homelessness and substance use.
• Ability to collaborate across Glide programs and city agencies.
• Knowledge of harm reduction principles and risk reduction counseling.

Responsabilités

• Outreach, educate, and link Glide HEAT clients to MOUD medications and other resources.
• Maintain a caseload of 20–30 participants and support medication adherence and housing resources.
• Link clients to primary health care and mental health services; assist with documentation and data entry.
• Develop outreach materials to engage the broader community in MOUD programs and coordinate events.
• Participate in HEAT program services and collaborate with Glide teams to improve client outcomes.
