Job summary

GLIDE is seeking a MOUD Case Manager to connect participants with medications like methadone, Suboxone, and naltrexone, and to support stabilization. You will manage a caseload of 20–30 clients, focusing on treatment adherence, housing resources, and access to mental health services.

The role emphasizes harm reduction, collaboration with Glide HEAT programs, and strong engagement across city agencies. A bachelor’s or equivalent MOUD experience is required, with HIPAA and ethical case management

Qualifications

• Bachelor’s degree in social work, public health or related field OR 2+ years MOUD navigation experience.
• Experience with overdose reversal, de-escalation, motivational interviewing.
• Ability to build relationships across Glide programs and with city agencies.
• Understanding of social determinants of health and homelessness.
• Familiarity with HIPAA and case management ethics.

Responsibilities

• Outreach, educate, and link Glide HEAT clients to MOUD medications and services.
• Maintain a caseload of 20–30 participants, promoting adherence and stabilization.
• Assist with housing resources, identification, mental health access, and documentation.
• Document and maintain complete client files and submit monthly activity reports.
• Coordinate outreach events and participate in HEAT program services and team meetings.

Education

Bachelor’s degree in social work, public health or related field OR 2+ years MOUD navigation and harm reduction case management experience
