Job summary

MBK Senior Living is hiring a Health Services Coordinator/LPN to support the health services team. You will assess potential residents, draft and update service plans, and coordinate medication delivery under the Director of Health Services guidance.

The role requires an active RN/LVN/LPN license and 2+ years of related healthcare experience. You will work in a full-time capacity, ensuring safety, compliance, and high-quality resident care in a supportive community.

Qualifications

• RN, LVN or LPN license active and in good standing; CEU and renewal requirements.
• 2+ years of related healthcare experience in a similar environment.
• Strong communication, decision-making and organizational skills.

Responsibilities

• Draft initial individualized Service Plans and update as needed.
• Review service plans with Executive Director as requested by DHS.
• Coordinate medication delivery and maintain accurate documentation in MARs.
• Provide coverage of duties in absence of team members and assist in training staff.
• Maintain a safe environment and ensure regulatory compliance.
