Job summary

Heritage Communities, The Heritage at Sterling Ridge in Omaha, seeks a Health Services Coordinator (LPN or RN) to improve resident well-being. You will assess needs, coordinate care plans, and support regulatory readiness while building strong relationships with residents, families, and partners.

The role requires active nursing licensure, CPR/BLS, 1+ year supervisory experience, and the ability to transport residents. Strong communication and tech skills are essential.

Qualifications

  • Active state nursing license (LPN or RN) with clean record.
  • Current CPR/BLS certification.
  • 1+ year supervisory/leadership in senior living.
  • 1+ year care for cognitive/memory-care residents (preferred).
  • Valid driver’s license with acceptable driving history; able to transport residents and assist at events.
  • Ability to lift/push/pull up to 50 lbs and assist residents with limited mobility.
  • Strong communication skills and ability to collaborate with diverse groups.
  • Proficient use of Word/Excel/PowerPoint and maintain HIPAA confidentiality.

Responsibilities

  • Conduct comprehensive resident assessments pre-move-in and ongoing.
  • Support regulatory readiness: accurate service plans, med records, and emergency prep.
  • Establish relationships with residents, families, staff, and external partners.
  • Coordinate health-related training for caregiver team; observe skills.
  • Provide direct resident assistance as needed, including personal care and medications.

What You’ll Do:

As a Health Services Coordinator (LPN or RN), you’ll play a hands-on role in improving the lives of our residents by:

  • Conducting comprehensive resident assessments prior to move-in, re-entry, and on an ongoing basis to ensure all physical, mental, and social needs are met.
  • Assisting with community readiness for regulatory inspections, including maintaining accurate service plans, medication records, and emergency preparedness.
  • Establishing and fostering positive relationships with residents, families, associates, home health, hospice, and other external partners.
  • Coordinating and ensuring proper health-related training and education for the caregiver team, conducting skill observations to optimize resident care delivery.
  • Providing direct resident assistance as needed, including personal care activities and administering medication in accordance with physician orders.
Requirements
What We’re Looking For:
  • Must hold active state-required nursing licensure (LPN) and an acceptable record.
  • A current CPR/BLS certification.
  • 1+ years of experience in a senior living or related industry supervisory or leadership role.
  • 1+ years of experience providing care to cognitive and memory-care residents or patients (preferred).
  • Valid driver’s license and acceptable driving record; ability to provide resident transportation and/or serve alcoholic beverages at community events.
  • The ability to assist residents with limited mobility, lift/push/pull up to 50lbs.
  • Excellent verbal and written communication skills, with the ability to effectively collaborate with diverse groups, navigate challenging situations, and make sound decisions.
  • Strong technical skills (e.g., Word, Excel, PowerPoint) and the ability to maintain confidential and privileged information (including HIPAA, residents, and associates).
