Job summary

Source One Management Services, LLC is seeking a Nurse Practitioner to perform in-home Health Risk Assessments for Medicare Advantage members. You will travel locally, review medical histories, and conduct basic health evaluations, enabling autonomous work in a flexible PRN role.

As a 1099 contractor, you can set a schedule with an 8-day minimum per month, use a company EMR, and complete onboarding in 3–4 weeks. This position offers competitive pay with incentives and no on-call duties.

Qualifications

• Active NP license (excluding Pediatrics, PMHNP and Women’s Health)
• Minimum availability of eight full 7.5-hour days per month
• Willingness to travel within roughly 60 minutes of your home
