Source One Management Services, LLC is seeking a Nurse Practitioner for a flexible PRN in-home Health Risk Assessments for Medicare Advantage members. You’ll work autonomously, set your schedule with a monthly 8-day minimum, and complete documentation on an iPad provided by the company.

Role includes malpractice insurance and collaborating physicians. Travel locally to patients in your county and nearby areas, with no on-call shifts or prescribing responsibilities.

Qualifications

• Active NP license in Kentucky required; excludes Pediatrics, PMHNP, Women’s Health.
• Ability to travel within roughly 60 minutes of home.
• Minimum availability of eight 7.5-hour days per month.

Responsibilities

• Conduct comprehensive in-home Health Risk Assessments for Medicare Advantage members.
• Review patient medical history, medications, lifestyle, and social factors.
• Perform basic physical and mental health evaluations; document via EMR.
