Job summary

Source One Management Services, LLC seeks a Nurse Practitioner for a flexible PRN in-home Health Risk Assessments for Medicare Advantage members. You will travel locally to patients, perform comprehensive HRAs, and review medical history, medications, lifestyle, and social factors.

Qualifications
