Job summary

Source One Management Services, LLC is seeking Nurse Practitioners for flexible in-home Health Risk Assessments (HRAs) for Medicare Advantage members. This PRN role offers autonomy, a simple iPad-based workflow, and the chance to supplement income without ongoing patient treatment responsibilities.

You will conduct comprehensive in-home HRAs, review patient history, meds, and lifestyle, perform basic health evaluations, and travel locally within your county and nearby areas.

Qualifications

• Active NP license required (excluding Pediatrics, PMHNP and Women’s Health specialties).
• Minimum availability of eight full 7.5-hour days per month.
• Willingness to travel within roughly 60 minutes of home.
