Job summary

La Clinica Center for Learning and Innovation in Medford, OR seeks a Health Records Clerk to organize and manage health information, maintain the integrity of the legal health record, and support the care team with medical, dental, and behavioral health records.

Responsibilities include filing and indexing records, creating digital copies, protecting confidentiality, processing release requests, and coordinating communications via fax, mail, courier and MyChart messages.

Qualifications

• Minimum: High School Diploma.
• Excellent interpersonal communication and problem-solving skills.

Responsibilities

• Organize and manage health information data and ensure the integrity of the legal health record.
• Assist patients and other agencies with Medical, Dental and Behavioral health record needs.
• Sort, file, and collate health records into patient records per procedure.
• Create digital copies of paperwork and store records electronically per policy.
• Protect patient health records and maintain confidentiality.
• Process incoming/outgoing release requests for health records per HIPAA.
• Provide excellent customer service to external partners and internal teams; handle requests via telephone, email, and MyChart.
