Banner Health seeks a Health Partner to support Banner Aetna members with care coordination and case management. You will engage patients using motivational interviewing, manage complex cases, and connect resources to improve health outcomes.

The role is hybrid in the Phoenix metro area, with Monday–Friday 8:00AM–5:00PM availability and travel 2-3 times per month. A master’s in social work and LMSW/LCSW license are required.

Formación

• Master's degree in social work is required.
• LMSW or LCSW license with two years of case management experience required.
• Strong interpersonal and communication skills are essential.
• CCM certification is preferred.
• Travel may be required within the Greater Phoenix area 2-3 times per month.

Responsabilidades

• Manage high-risk and complex cases across the health care continuum.
• Serve as the main point of contact for members and providers.
• Coordinate care and connect resources to improve outcomes.
• Educate care teams on case management concepts and best practices.
• Document assessments and care plans in member records.
