Job summary

Banner Health in the Phoenix metro area is seeking a Health Partner to serve as a trusted resource and advocate for Banner Aetna members. You will proactively engage members, assess needs, connect resources, and help navigate their healthcare journey in a home-based care coordination role.

This hybrid position requires self-motivation, caseload management, and collaboration with care teams to ensure timely, coordinated care and improved health outcomes for high-risk and chronic conditions.

Responsibilities

• Manages patients across the health care continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes.
• Provides self-management support, including checklists and escalation as prescribed by protocols.
• Applies motivational interviewing to promote lifestyle changes and patient enrollment in case management.
• Establishes collaboration with physicians, payers, and care teams to fulfill utilization and regulatory requirements.
• Meets with members and families for initial appointments and home assessments to navigate the health care system.
• Educates internal care teams on case management concepts and integrates these into daily practice.
