Job summary

Johnson County Government's Adult Services Team is hiring a Health Navigator (MNH Case Manager) to work with individuals who have severe mental illness. The role provides short-term case management, helps access physical health services, behavioral health care, and links to community supports.

Duties include assessing needs, planning services, coordinating with community agencies, delivering services for a caseload, documenting progress, and overseeing the SOAR program.

Qualifications

• Bachelor’s degree required in human services or related field.
• Driver's License with acceptable driving record required.
• Johnson County Government requires reference/background screening for all positions.
• Education/Experience substitutions allowed.

Responsibilities

• Performs assessments to identify needs and determine eligibility for services.
• Develops plans to address needs and coordinates with community agencies.
• Provides short-term case management and services for designated caseloads.
• Manages the SOAR program (SSI/SSDI Outreach, Access, and Recovery).
• Outreaches to high-risk clients and documents progress toward measurable goals.
