Job summary

Pawnee Mental Health is seeking a dedicated Case Manager to assess clients, coordinate benefits, and link them with community resources across Kansas. You will work closely with the Case Manager, Social Security Administration, and other partners to ensure timely access to Medicaid/Medicare benefits and support services.

The role requires strong communication, organizational skills, and a willingness to travel within the 10-county area.

Qualifications

• Bachelor’s degree in human services or equivalent work experience.
• Strong oral and written communication in English.
• Proficient with Microsoft Office and basic PC skills.
• Valid driver’s license and auto insurance.
• Ability to work with individuals with behavioral health needs.
• Excellent organizational and time-management skills.
• Case Manager experience preferred.

Responsibilities

• Meet with clients during comprehensive assessments and collect vitals.
• Assist clients with Release of Information forms and portal registration.
• Conduct follow-up calls three business days after assessment.
• Coordinate with Case Manager and providers to obtain benefits.
• Travel to serve clients across the ten counties as needed.
• Collaborate with staff on benefit acquisition and maintenance.
• Assist clients with Medicaid/Medicare applications and documentation.
• Maintain timely documentation in the EMR and other systems.
