Job summary

ACMH INC in New York, New York is seeking a Health Navigator/Care Coordinator for Care Management Services (CMS). The role is based in Manhattan with travel throughout Manhattan and the lower Bronx, and a Monday–Friday, 9:00am–5:00pm schedule. Responsibilities include assessing clients, developing comprehensive care plans, coordinating services, documenting interventions in the EHR, and supporting transitions of care with the care team.

Requirements: B.A. or M.A.

Qualifications

• B.A. or M.A. in social services or related field with two years of direct service experience.
• Strong written and verbal communication skills.
• Bilingual English/Spanish preferred.

Responsibilities

• Assess and engage clients around health and wellness and develop care plans.
• Administer health screenings and identify interventions.
• Document interventions in the EHR and coordinate with the care team.
• Support transitions of care and arrange transportation as needed.
