Job summary

ACMH, Inc. in Manhattan seeks a Health Navigator/Care Coordinator to assess clients, engage them in health/wellness, and develop comprehensive care plans. The position requires travel across Manhattan, Queens and the Bronx and a Monday–Friday, 9:00am–5:00pm schedule, reporting to the Director of Care Management Services.

B.A. or M.A. in social services or related field with two years of direct service experience is required; strong written and verbal communication skills are essential, bilingual

Qualifications

• Bachelor's or MA in social services or related field.
• Two years of direct service experience.
• Strong written and verbal communication skills.
• Bilingual English/Spanish preferred.

Responsibilities

• Develop rapport with clients to engage them in improving health and wellness.
• Administer standardized health and psychosocial risk screenings per protocol.
• Identify interventions and develop a comprehensive care plan.
• Collaborate with care team to address needs and optimize outcomes.
• Coordinate care by scheduling appointments and arranging transportation.
• Document interventions and contacts in the EHR.
• Participate in team meetings and clinical conferences.
• Advocate for client goals and connect to community resources.
