ACMH INC in New York, NY seeks a Health Navigator/Care Coordinator to assess clients, engage them in health and wellness, and develop comprehensive care plans. Location is Manhattan with travel throughout Manhattan, Queens and the Bronx. Schedule is Monday–Friday, 9:00am–5:00pm, with a focus on coordinating care and evaluating outcomes.

The role reports to the Director of Care Management Services. Requires a BA/MA in social services with 2 years of experience; bilingual English/Spanish

Qualificações

• B.A. or M.A. in social services or related field.
• Two years of direct service experience in human services or CM/Service Coordination.
• Strong written and verbal communication skills; bilingual English/Spanish preferred.

Responsabilidades

• Develops rapport to engage clients in improving health and wellness.
• Administers standardized health and psychosocial risk screenings.
• Identifies interventions and develops a comprehensive care plan.
• Collaborates with care team to achieve optimal health outcomes.
• Documents interventions in the EHR per program standards.
• Performs home visits to assess living conditions and needs.
• Coordinates appointments and transportation for clients.
