Job summary

Altais Health Solutions is seeking a Health Navigator to join our Berkeley-based team onsite, supporting clinical staff with care coordination and disease management. You will help members access medical, social, and behavioral services, and document care coordination activities in our systems.

The role emphasizes strong communication, collaboration with providers, and the ability to handle multiple priorities in a fast-paced environment.

Qualifications

• High school diploma or equivalent required.
• Minimum 2 years in an acute care facility, preferably in case management or emergency department.
• Preferred: Medical Assistant, CNA, Home Health Aide, or similar certification.
• Strong organizational skills.
• Strong verbal and written communication skills.
• Ability to cope with stressful situations and manage multiple priorities.

Responsibilities

• Provide support to participants in case and disease management programs to meet treatment goals in coordination with case managers.
• Support members in navigating and connecting to clinical and community resources.
• Assist patients to connect to health screening programs and resources.
• Document care coordination, care gaps, and discharge planning needs in care management systems.
• Assist clinical staff in outreach, orientation, and baseline assessment services.
• Establish and maintain relationships with participants, caregivers, and PCPs/Providers, coordinating with resources as needed.
• Provide one-to-one guidance, support, education, and coordination of care as they move through the healthcare continuum.
• Participate in case conferences and CM team meetings to support care coordination.
• Educate and answer inquiries about benefits, services, eligibility and referrals with a professional approach.
• Develop/update member education materials and mailings when appropriate.
• Identify and provide resources and community referrals to address presenting issues and barriers.
• Assist members in getting appointments and access to care and program services.
• Initiate follow-up to coordinate additional needs across care settings.
• Participate in interdisciplinary collaboration with CM team and external partners.
