Job summary

Alameda Health System's SLH Care Management team in San Leandro seeks a Community Health Worker to provide care coordination and advocacy to patients, linking them with resources and services to support health and wellbeing.

You will conduct risk assessments, coordinate with care teams, arrange transportation, manage documentation, and help patients navigate community programs, with a focus on social determinants of health.

Qualifications

• Bachelor's degree in a health/social field preferred.
• Three years of related community-based health care experience preferred.
• Valid California driver’s license required.

Responsibilities

• Administers intake assessments and records in the patient’s medical record.
• Analyze client activities and coordinate appropriate program activities and resources.
• Coordinate patient care activities with the health care team and community partners.
• Assist with documentation, referrals, transportation, and scheduling to support care delivery.
• Screen for barriers to care and provide counseling on resources and services.
